About Adult Health Home Care Management - St. Mary's

This program is available to adults with active Medicaid, and is designed to help people who may have ongoing chronic health issues, behavioral health needs, and/or addiction treatment needs. The healthcare and service providers at Health Home Care Management will help you receive the services you need.

We can help if you experience any of the following:

• a chemical addiction condition
• a mental health condition
• a chronic physical condition
• difficulty finding a provider or getting to your appointments
• frequent visits to the ER
• unsafe living conditions
